We need to verify your identity in order to provide you a MyChart Activation Code. If you have scheduled or completed an appointment with Cleveland Clinic London in the last 30 days, you will have received a MyChart Activation code. This will be shown in your email appointment confirmation or it will be contained in the After Visit Summary given to you after your appointment.
